Community Health Planning
Community Health Assessment 2016 cover

Since priority health issues in a community constantly change, it is important to monitor the health of the community on a regular basis and revise the focus of improvement activities, as needed.

A community health assessment is performed in collaboration with community members to learn about the Northern Kentucky community. Community health assessments describe the health of the population, identify areas for health improvement, identify contributing factors that impact health outcomes, and identify community assets and resources that can be mobilized to improve population health.

The Community Health Assessment for Northern Kentucky, published in May 2016, identifies key health priority issues in Northern Kentucky. The results of the Community Health Assessment are used to provide direction for interventions, policy formation, implementation and evaluation found in the Community Health Improvement Plan.

Community health planning is a critical part of the Public Health Accreditation Board's requirements for accreditation.

Prior planning work

Since 1993, Northern Kentucky has been a leader in community health planning. Northern Kentucky was the first health department in Kentucky to fully utilize the Assessment Protocol for Community Excellence in Public Health (APEXPH) process developed by the National Association of County and City Health Officials in 1991. By using this process, Northern Kentucky developed the Community Health Plan 1996. Repeating APEXPH with the addition of being a pilot site for the Protocol for Assessing Community Excellence in Environmental Health, the Community Health Plan 1999 was developed.


In 2000, the Health Department was selected by NACCHO as one of nine national demonstration sites for the Mobilizing for Action through Planning and Partnerships process. This process resulted in the Public Health System Improvement Plan 2003. These three plans and other community led assessments were combined into the Master Health Plan for Northern Kentucky 2005. In 2010, the Health Department completed a second MAPP process to implement Vision 2015's livable community strategies, and published the Vision for a Healthy and Vibrant Community in 2010.

Compiled by the Health Department in 2015, the 20-year analysis of health trends was an attempt to evaluate long-term health outcomes as a result of the previous community health improvement plans and provided a foundation for the 2016 community health assessment and planning process.
